After opening up the Sitecore Experience Forms UI and select selecting the create new form option, you can add fields to the form that you want to be mapped to a field in a Salesforce object.

In the above example we may intend to map the first name, last name, email address and birthday fields to, for example, a Salesforce Contact. Once all the fields have been added, click Submit to show the Submit Actions on the bottom right side of the form. Add the Submit Action called “S4S Generic Submit Action” from the list .

If analytics are going to be pushed to Salesforce the S4S Generic Submit action can be configured to save form fields into the corresponding xDB contact so that they show up in the Sitecore Experience Profile screens. Select the S4S Generic Submit Action action and click on the "Edit" button

Set the form fields that will be used to update the xDB contact. The first name and last name will be stored in the Sitecore Personal Information facet. The email will be stored as the preferred email in the Sitecore Emails facet. Leave the field name blank if data is not to be saved in that particular Sitecore facet.
